	E-mail the form but send the payment by mail. We do not accept online payments at this time.
	Make the cheque payable to the Canadian Slovak/ Kanadský Slovák and mail it to: Kanadský Slovák 259 Traders Blvd. East, Suite 6 Mississauga Ontario L4Z 2E5
	Your personal login and password to access the e-newspaper will be sent to your e-mail address once your payment is processed.

	Print out the Printable Registration Form.
